Nursing Bears can help participants manage hospital discharge and ensure that this is timely discharge with the right NDIS-funded supports.
This usually involves four areas to improve discharge and outcomes:
- Working with NDIS supports,
- Finding out the participant’s housing needs and preferences,
- Exploring housing solutions, and
- Transitioning to interim and long-term housing
Nursing Bears places a high priority in enhancing outcomes for people with newly acquired disability and/or change in condition who are NDIS participants or NDIS eligible.
Some of the barriers that Nursing Bears can help overcome are health facility staff knowledge of the NDIS processes, understanding the processes and timeframes of the NDIA, timely access to the right NDIS funding for supports, assistive technology and SDA funding, and lack of understanding of SDA funding and suitable housing market options.
This will result in timely access to the NDIS, early approval, and developing transitional arrangements for support and housing where required.
